SMC-L corresponds to 38 U.S.C. § 1114. It pays a higher total rate instead of the basic rate. Qualifying conditions: anatomical loss or loss of both feet, or one hand and one foot, or bilateral blindness (5/200), or permanently bedridden or in need of regular aid & attendance. 38 CFR § 3.350 and 38 CFR § 3.352 define the rules.
Loss criteria: both feet amputated or useless; or one foot + one hand; or blindness 5/200; or permanently bedridden. Aid & Attendance: any veteran needing assistance with daily tasks may qualify. Must be so helpless they need help bathing, dressing, etc. 100% schedular is not strictly required by law.
Medical evidence of amputations or blindness. For A&A: VA Form 21-2680 (Physician's Report) documenting need for assistance. Personal statements or nurse reports describing inability to perform ADLs. Doctor's certification for bedridden status.
File VA Form 21-526EZ or write a letter claiming SMC aid and attendance or housebound. Submit VA Form 21-2680 to document A&A or Housebound status. Specify bedridden or A&A in the claim. Include daily diary or buddy statements if helpful.
