Download the correct DBQ from VA's public DBQ list.
Have your health care provider complete it. VA's DBQ guidance instructs claimants to have their provider fill out and submit the appropriate DBQ.
Ensure the provider block is complete. VA's fraud-prevention guidance says private clinicians must complete provider info at the bottom and sign/date the DBQ.
Submit it as supporting evidence with your claim (or to support an increase). VA explains the types of evidence needed for different claim types and that you can submit medical records/opinions and lay evidence like buddy statements.
Expect that VA may still schedule an exam. VA may require an additional exam to complete the claim, and regulations authorize an exam if evidence isn't adequate for rating purposes.
