VA publishes a central list of Public Disability Benefits Questionnaires (DBQs) organized by body system (for example cardiovascular, musculoskeletal, respiratory, and mental health).
Use these DBQ PDFs to help your health care provider document the details VA needs for your claim. VA says DBQs can be used to submit medical evidence from your providers and that your provider should fill out and submit the appropriate form.
